请教各位妈妈，孩子打疫苗的小本，就是介绍孩子什么时候打什么疫苗的一本书是在哪里领啊，谢谢！我住在武吉巴督。

出院的时候医院给的，里面有记录医院出生时的体重身高等信息，你的医院没有给你吗？如果不是本地出生那就去打针的地方，我想应该是会有吧！这个本本很重要，以后上学什么的都需要。

去你家附近的Polyclinic问问看，他们那里应该有那个小册子。如果你在其他地方有打疫苗记录，就带上，他们会比较一下，然后填写到新的小册子上。
